What fees and commissions does the broker charge?

Question
What fees and commissions does the broker charge?
Notes
Generally brokerage is charged at a flat rate per trade, but if you trade over a set limit, it can be charged as a percentage e.g. 0.12% for online trades over $25,000. Generally, fixed rates for online brokers are around $30 for trades up to $25,000. Whereas phone brokers usually charge a bit more i.e. ~$70 for a large trade. However, they can start from around $30 for trades under $5000.
